2012 Chronic Disease Self-Management Update
The Stanford Patient Education Research Center offers web-based update training for the new 2012 Chronic Disease Self-Management Program (CDSMP) to those who have previously been trained as a Master Trainer or T-Trainer in the Chronic Disease Self-Management Program.
CRITERIA FOR TAKING THE UPDATE TRAINING (At least one of the following conditions must be met)
1. Be an active certified CDSMP Master Trainer or T-Trainer
- An active Master Trainer is one that has conducted at least one Leader Training within the 18 months prior to update enrollment date.
- An active T-Trainer is one that has conducted at least one Master Training or Leader Training within the 18 months prior to update enrollment date.
2. Someone trained within 6 months prior to update enrollment date whose certification is in progress.
3 .Someone trained within 6-12 months prior to new CDSMP roll out date whose certification is in progress.
2010 Arthritis Self-Management Update
The Stanford Patient Education Research Center offers web-based update training for ASMP Leaders. This is the only format for updating ASMP Leaders in the U.SA. . Master Trainers and T-Trainers outside of the U.S.A. may also take the web-based update.
CRITERIA FOR TAKING THE UPDATE TRAINING
Trainee must be an active certified ASMP Leader. An active Leader is one that has conducted at least one ASMP workshop within the 18 months prior to update enrollment date.
What is the format for a web-based training?
You will be asked to complete pre-webinar assignments, and attend 1 real time webinar with your T-Trainer and up to 25 other trainees. Participants will meet together through the webinar meeting for 2 hours (arthritis) or 3 hours (CDSMP).
How do I access the pre-webinar activities?
The update training requires that you spend time reviewing training videos and completing training assignments before the webinar. All training activities and videos can be found on the website. After you have registered, you will receive a username and password to access the training site about 2 weeks prior to the webinar. That email will also contain details about the times and dates for the webinar and for homework deadlines.
How do I access the webinar?
We will send you an invitation via email to access the webinar. This email will contain the dates and times as well as any other instructions that you will need. There is also an instruction form that you can download from the update training site.
How do I register for update training?
A link to the trainings and the registration form is found on the menu on the left. The registration site will open in a new window.
What if I know someone else that wants to attend?
In order to get credit for attending an update training, each trainee needs to register for the training. Send them the link to this page.
Once I am updated for the 2012 CDSMP, can I update CDSMP Leaders using the web format?
No. Only Stanford trainers can conduct web-based update trainings. Updating CDSMP Leaders must be done face-to-face. You will receive an update manual for the 1-day update Leader Training.
Once I am updated for the 2010 ASMP, can I update ASMP Leaders using the web format?
No.The ASMP update training is for Leaders, and only Stanford trainers may update U.S.A. Leaders. There are no longer ASMP Master Trainers in the U.S.A.
Master Trainers outside of the U.S.A. may update Leaders, but not using web-based training. Leaders outside of the U.S.A. must be updated in a face-to-face training.
